CPQCC offers two main NICU databases for tracking infants:
1. Higher Acuity Database
Tracks Small Babies (<31 weeks, 6 days gestation or <1,500 g) and Big Babies with specific severe conditions or procedures.
Includes infants born at your hospital or transferred in within 28 days of birth and admitted to the NICU.
Collects detailed information via the Admission/Discharge form, CPeTS form, or Delivery Room Death form.
Vermont Oxford Network (VON) eligibility: Infants with birth weight <1,500 g or gestational age <29 weeks, 6 days are automatically uploaded to VON; CPQCC manages eligibility on the back end.
2. All NICU Admissions Database (ANAD)
Tracks all infants admitted to the NICU, regardless of size, gestation, or condition.
For higher acuity infants, members either import from the Higher Acuity NICU Database or start with the ANAD form to partially populate the Higher Acuity Database.
Data can also be uploaded/downloaded via .csv, and CCS Form sections A-F can be partially populated.
Captures infants not eligible for the main NICU Database but who affect NICU resource use and reporting.
