Q: Is there a way to look up a hospital by name or hospital ID for the CPQCC A/D and CPeTS forms?
A: Yes. In the online A/D and CPeTS forms on cpqccdata.org, you can use the Narrow List tool included with all hospital drop-down selection boxes. The hospital list is sorted alphabetically by name, and you can search by any part of the hospital name or by the hospital’s HCAI ID.
In addition, CPQCC provides a HCAI Location List with all active California birth hospital information on the Data Resources webpage, which can be used as a reference when completing forms.
