Q: Is there a way to look up a hospital by name or hospital ID for the CPQCC A/D and CPeTS forms?
A: You can use the Narrow List tool that accompanies all drop-down hospital selection boxes, in the online A/D and CPeTs form at www.cpqccdata.org. The list on the form is sorted in alphabetical order by hospital name. You can use the search function built into the drop-down list to search for any name part of the transport location, or you can use the transport location's HCAI ID.
