Q: Who is responsible for referring HRIF eligible infants?

A: It is the responsibility of the discharging to home California Children's Services (CCS) Neonatal Intensive Care Unit (NICU)/Hospital OR the last CCS NICU/Hospital providing care to make the referral to the High Risk Infant Follow-Up (HRIF) Program.



Q: What is the HRIF ID Number? 

A: The HRIF ID Number is a computer generated 8-digit number assigned to the infant/child after submitting the "RR Form" in the web-based Reporting System.



Q: How can I search for Records in the System?

A: Use the "Quick Patient Search" page to find patients by using any of the following variables:

  • HRIF I.D. Number
  • CCS Number
  • NICU Record ID
  • Hospital Specific Medical I.D. Number
  • Infant's Last Name
  • Infant's First Name
  • Infant's Date of Birth
  • Infant's Birth Year
  • Birth Hospital
  • Birth Mother's Date of Birth

Q: When do I use the "Still in Hospital" check box?

A: If the infant/child is still hospitalized in the NICU or other unit in the hospital at 8-months chronological age.



Q: How do you obtain a NICU Record ID?

A: Every CCS-approved NICU hospital has a NICU Data Contact that keeps a record of all CPQCC NICU-eligibility patients. Use the "NICU / HRIF Directory" to identify the CPQCC Data Contact(s) from the discharging/referring or birth hospital. The directory is under Admin -> Update Directory -> Download Directory in the HRIF Reporting System.




Q: An infant has two NICU Record IDs, which one do I use?

A: The OSHPD facility code and NICU Record ID must match. If you use the birth hospital's OSHPD code then you must use the birth hospital's NICU Record ID. 



Q: What if I don't know if the infant qualified for a NICU Record ID?

A: Enter "00000" for infants when a NICU Record ID has not been assigned at the time of the infant’s referral to the HRIF Clinic. If a NICU Record ID is assigned to the infant later, use the Manage NICU Reference ID tool to correct the NICU Record ID. Use the "NICU / HRIF Directory" to identify the CPQCC Data Contact(s) from the discharging/referring or birth hospital to verify if the patient qualified for a NICU Record ID. The directory is under Admin -> Update Directory -> Download Directory in the HRIF Reporting System.




Q: Do I still have to refer infants who meet HRIF criteria, but will not be followed in California because the family lives out of state/country?

A: Yes. Please submit a Referral/Registration (RR) form and then complete a Client Not Seen Discharge (CNSD) form and indicate the family moved out of the state/country, then close the form.



Q: Patient seen for 1st visit and scheduled to return, but 6-8 months have passed and the patient has not been seen after multiple (3-5) attempts. How would I code this in the system?

A: Submit a CNSD form using the date of the last attempt to contact, Reason = Unable to Contact and Disposition = Discharge-closed out of Program.



Q: When do I use the "Unable to Complete Form" check box?

A: Use only when the HRIF Clinic is unable to complete the Referral/Registration form, because the infant/child was lost to follow-up, expired prior initial visit or the primary caregiver(s) refused to follow-up service. Then submit a "Client Not Seen/Discharge (CNSD) form" to indicate the reason you are unable to complete the form: "Infant expired", "Lost to Follow-Up", or "Parent Refused".
